Is there a way to make it so that a customer has to select a reason when someone cancels their appointment? I would love to be able to get some feedback when they cancel even if it is just a form with a few radio buttons and text field.

Currently this is a feature request. I can totally see how feedback could provide insights on why a customer canceled. I will make sure to share this request with our Product Team. I've gone ahead and moved your post to the features request section as well.
