Select multiple items in a form using check boxes

I probably asked this question before, but unfortunately here I can only see my latest posts.
How can I have multiple options clicked in the form? I know, I have to use check boxes, but I can find check boxes anywhere.

Question: how can I find and use check boxes in a form?

Hi @JoeRBT The option should be on the left side menu. Can you please confirm the site name and page name? I'll take a look at your current form setup. Thanks!

I am unable to create check boxes. I was able to do it before. But, now I can't figure out how to find the check boxes on the left side menu.

Please kindly advise.

Thanks for confirming. Did you add the Contact Form element first? Once you drag/drop the contact form onto your page and click inside to edit, you will see the Checkboxes option on the left.
