Hi there,

I am trying to create a wedding website and I would like to have an RSVP form that requires guests to first look up their name, and then a form will pop up and only include yes/no to the guests in their party. 

I.E. 

Name: James Smith then form James Smith (check box yes or no) and Judy Smith (check box yes or no).
